Pathways Hospice Receives $2K Grant From Los Altos Community Foundation

The Los Altos Community Foundation (LACF), funded by the David and Lucile Packard Foundation, awarded a $2,000 grant to Pathways Home Health & Hospice. 

The grant will help Pathways’ provide care for those in the community with life-limiting illnesses.

“Pathways Hospice has close ties within the Los Altos community,” said Roy Lave, Executive Director of the Los Altos Community Foundation. 

“One of the primary goals of LACF is to strengthen the non-profit sector and, by doing so, build a stronger community.  We are proud to support them in their mission of caring for neighbors who are facing a critical time in their lives.”

Pathways Home Health, Hospice & Private Duty provides a family of health services in the home for patients including home health, hospice care, private duty non-medical attendant for personal care, geriatric care management services, grief support, and pediatric hospice care for children with life-threatening illnesses. 

“Pathways Home Health & Hospice has been part of the Los Altos community since we were founded in 1977,” said Sandra Coolidge, Vice President of Admissions, Community Relations and Philanthropy at Pathways.  “We are very appreciative of this grant from the Los Altos Community Foundation, and will use the funds to continue to provide quality care for those in need.”
